Filling out a Form

Entering Text Into a Form Box


1. Place the cursor inside the box next to First Name and click - You should see a blinking cursor line inside the box.

    Type your First Name

2. To get to the next box, you can place your cursor in that box and click, OR you can use the "tab" key to jump to the next box.

3. To complete a date, enter the information in the following format: dd/mm/yyyy

Answer the following questions

First Name
Last Name
Today's Date
Street Address
City
 

 


Using Radio Buttons

Radio Buttons are used when only 1 choice is wanted.

1. Click in one of the circles in the list below

2. Click in a different circle

Choose one of the following options:

I feel very confident using a computer
I feel somewhat confident using a computer
I am still a little confused
I am totally lost
I want to throw my computer in the trash

Notice that when you chose the second button, the first one automatically unselects..


Using Check Boxes

Check boxes are used when one or MORE choice can be submitted.

1. Choose several of the answers below by clicking in the boxes.

2. Click again, to make the check mark disappear.

Notice that you can click in one or all of the boxes and they stay selected.


 

Using a Drop-Down Menu

Click on the down arrow.

1. Highlight an option and left click

2. To change your choice, repeat this procedure.
